When an approximately twenty by sixteen foot wall of dampers was sucked into a 150 hp air handler for the second time, the mechanical and general contractors at the Charleston Air Force Base realized that they needed third party commissioning to find the reason for these catastrophic failures. They wanted to ensure that nothing like this occurred again at the new facility.
Our analysis showed that only software interlocks, which could be overridden, were being used. We recommended hard wiring interlocks and recommissioning the controls systems to ensure proper operations.
After several years of continuous operations, there have been no further repeats of this catastrophe. |
